Workplace injury incidents have been on a steady decline in Alaska over the past two decades, but workers’ compensation costs continue to rank as the most expensive in the nation driven by insurance rates tied to high cost of care. Photo/File/AJOC June 23, 2013 A team from the Alaska Workers Compensation Board traveled the state June 17 to 20 holding public meetings to discuss rising medical costs associated with on-the-job injuries. At our main meeting our board decided it wanted to get out in the community and get feedback from employers, employees, from doctors, from attorneys from the stakeholders in the workers compensation system, state Division of Workers Compensation Director Mike Monagle said at the June 18 Anchorage meeting. Other meetings were held in Fairbanks, Kenai and Juneau. Alaska s total workers compensation system costs were $270 million last year, according to division statistics. A biannual study done by the state of Oregon found that Alaska carries the highest workers compensation insurance premium rates in the country, despite a 3 percent drop in premiums in 2012. In 2000, Alaska ranked 28th in workers compensation premium rates, but by 2006 it was the most expensive state in the country. Since then, Alaska has been first or second. Monagle said rising insurance costs are not related to incident rates, which have dropped from more than 30,000 reported workplace incidents in 1994 to less than 20,000 last year. Over the same period employment grew by nearly 90,000 jobs to roughly 320,000 statewide, he said. Employment s going up, (incident) frequency rate is going down. This frequency rate is really important because it s the only thing that has been keeping Alaska s premium rates in check,<a href="http://www.nfljerseysforfans.com/">Cheap NFL Jerseys</a>, Monagle said. The Oregon study found Alaska s premiums to be 160 percent of the national median. Premium rates for workers compensation insurance tend to be cyclical, Monagle said, and nationally rates have increased over the last two years. So the 3 percent in-state decline may not become a trend. Despite being cyclical, rates never return to their previous bottom, he said. Monagle reported that nearly 75 percent of total lost costs in 2012 associated with benefit claims were medical costs. Of the $260.7 million paid out in Alaska during 2011, $160.4 million, or 62 percent, was for medical benefits. Nationwide, medical benefits make up 59 percent of total costs, with indemnity, or lost work time accounting for the remainder, Monagle said. He called the annual 8 percent-plus increase in premiums nationwide unsustainable. Today, in all states, medical is the big cost driver, he said. Monagle broke medical costs down into three categories: fees, over-utilization and prescription drugs. He said specialty medicine costs are disproportionately high in Alaska. This includes treatments such as physical therapy and chiropractor visits. Some states have mitigated medical fees by implementing fee schedules, which help regulate what clinics and hospitals charge. Most states that have fee schedules are on the lower end of the cost spectrum, Monagle said. Alaska, which has a fee schedule, doesn t follow the trend. He likened a fee schedule to car shopping. You don t go into a car dealership and offer to pay full retail, nobody does, Monagle said. High treatment cost and high insurance premiums go hand-in-hand, he said. He declined to offer specific suggestions for further regulatory measures to curb premium costs but said Montana, which had been battling Alaska for the highest workers compensation costs in the country, recently instituted legislation that dropped it in the rankings. In 2010, Montana was first in total cost and by last year it had dropped to eighth, according to the Oregon study. Other states have also looked at ways to mitigate the over-use of expensive treatments to mitigate medical costs, Monagle said. Most states are moving towards utilization guidelines, he said. For example, for this diagnosis, this is the recommended treatment. He added that many of the doctors his division consults with do not receive workplace injury treatment training, thus they don t view workplace injuries differently. Monagle gave the example of an individual with a broken ankle being told to stay home for several weeks to allow the injury to heal. While that may be an effective treatment, he said, it is possible someone in that situation could receive a boot or air cast and return to work in some capacity off their feet and save thousands of dollars in indemnity costs. Monagle said costs associated with prescription drugs account for 20 percent of medical benefits paid in compensation claims. He said strong, opioid painkillers such as Oxycodone can have a debilitating effect on injured workers prescribed them for intense pain. Monagle said the drugs were never intended for general use, rather they were designed as end of life treatment. The drugs are of particular concern in workers compensation cases because patients are highly susceptible to dependency and subsequent depression and other mental health issues, Monagle said. While a worker may be physically healed, they are then mentally unfit for work, he said. Monagle noted programs that require patients who have been on opioids for more than 60 days without signs of physical improvement to enter treatment programs to prevent dependency. On the flipside, several states have also instituted drug testing to assure patients prescribed powerful painkillers are actually taking them and not selling them on the street for $100 a pill, Monagle said. Alaska does not have such programs. He cited state figures that estimate prescription painkillers account for 40 percent of all drug costs in Alaska. To compete with the new wireless player Verizon, Alaska Communications and GCI are trying to merge their infrastructure into the Alaska Wireless Network, or AWN. If approved, the two propose to continue marketing and selling separate products, but using the same network to provide their service. The company will be two-thirds owned by GCI and the rest by Alaska Communications, with GCI paying $100 million to its fellow Anchorage-based telecom. The companies have asked the Federal Communications Commission for expedited consideration of the request so that they could take advantage of the 2013 summer construction season. And, at least for now, ACS and GCI receive some roaming and backhaul activity from Verizon customers, which they have said they anticipate continuing on the AWN.
JUNEAU (AP) As the sun creeps into the sleeping quarters of the Tustumena, passengers who take a second to look out the window wake up to volcanic topography, sky blue lakes and wildlife that looks extraterrestrial even to most Alaskans. That ethereal experience only lasts a few seconds; the berth s stripped-down bunk beds and dreary wallpaper quickly remind passengers they are sailing on a ferry that is almost 50 years old. Having a ship that s a vestige of another era, however, does offer one small perk: During the summer, a handful of vessels have a nature expert on board who teaches passengers about the stunning local scenery and animals. Alaska s state-owned ferries which shuttle residents and tourists between remote towns on the coasts of Washington state, Canada and Alaska are scaling back costs by getting rid of the naturalist program on all but one of the 11-ship fleet this year. State officials say the program may eventually be brought back, but for now, the plan is to replace them with computerized equipment and brochures on the so-called Alaska Marine Highway System, which celebrates its 50th anniversary this year. In light of Alaska s declining revenues and an unclear financial future, the state s various departments were asked to bring expenses down by eliminating items that do not affect core functions. Naturalists, who are hired and paid by the U.S. Forest Service or the U.S. Fish and Wildlife Service, make about $22,000 a season. The state provides them free room and board on the ferry, which costs about $5,000 per year, per ship, according to Jeremy Woodrow, a spokesman for the Alaska Department of Transportation, the department responsible for the ferry system. The core purpose of the marine highway system really is providing transportation as a highway, Woodrow said. The Marine Highway system is an aging, bare-boned necessity. Only four of the 11 ships in the fleet were built after 1980, but they remain a crucial link connecting the state s coastal cities to the rest of the world. The only way to reach Alaska s capital, Juneau, for instance, is to fly or take a ferry. There are no roads in or out of the rugged terrain. Many of the ferries passengers are Alaska residents shuttling from town to town or back from the mainland. But the trips also draw adventuresome tourists looking for an off-the-beaten-path vacation. The naturalists, called interpreters, are a valuable tool for tourists and residents because staff members don t have time to play tour guide, according to Doug Stuart, who served as the Tustumena s naturalist for over a decade. Stuart, 71, is now out of work for the first summer in 12 years. He gets social security, but does odd jobs in the winter to supplement his income. Without the money from the naturalist job, Stuart and his wife are selling their big house in town with a mortgage where they currently live together after raising five children there and moving to a smaller one on the outskirts of town that he s been building for the last few years. Erin Kirkland, the publisher of AKontheGO.com, a website dedicated to family travel and outdoor activities in Alaska, said she is sad to see the naturalists go because a lot more tourists are starting to take the ferries instead of cruise ships. She and her family also enjoy the interpreters when they take the ferries. They have all the maps. They ve got all the information about the communities you re headed to, the national forests, the national parks, and they will offer very insightful information, she said. It s just a really nice fit. Interpreter programs on many ships began disappearing when funding from the federal government became less certain, Woodrow said. Without knowing for sure whether the federal government would be able to pay for interpreters in the future, the Department of Transportation is now hesitant to sign a contract to rent out a room for them. That s space that could be used to transport Alaska s tourists and in-state travelers, the department said. This summer will be the first time in 23 years that the Tustumena doesn t have a naturalist on board, according to U.S. Fish and Wildlife Service official Larry Bell. It s a loss for tourists because the state ferry system is the one of the few ways to see the Aleutian Islands. Cruise ships mostly travel southeast Alaska, with some venturing to Anchorage and Kodiak. Plenty of questions remain unanswered after senators lit into the two men, Steven Miller and Doug Shulman, about the agency’s managerial competence, the thoroughness of its investigation into the matter given that the IRS has yet to identify exactly who is responsible for the targeting, and whether IRS officials were less than forthcoming with members of Congress during 2011 and 2012 when lawmakers were looking into allegations that eventually proved true. When the IRS began implementing that rule half a century ago, however, they decided that exclusively really meant not “primarily” engaged in political activity, leaving the door open for groups like the Sierra Club or the AARP to claim the status but devote less than half their activities to politics. But figuring out just what counts as political activity and what’s an educational offering tied to social welfare is often in the eye of the regulator rather than a definitive science. Because the IRS, along with the Treasury Department, handles the regulations that spring from the tax code, “the IRS should be able to fix it on their own,” says Annette Nellen, a professor of accounting at San Jose State University. “The language in the statute does say ‘exclusively.’ ” Members of Congress look at the original law and say, like Sen. Yeast, and oats, and molasses, and butter, and flour; apron strings, and linoleum, and dough – these are the smells and feelings that my family knows instinctively as comfort and safety. They fill my small apartment kitchen when I open the oven and remove the warm, brown, slightly uneven loaves. I cut off an end piece and butter it up. This place has started to feel like home. Turn onto a lightly floured surface and knead. Shape dough into a ball and place it in the greased bowl. 7. Cover with a clean dishtowel and let rise in a draft-free place until double in size. 8. Place in greased loaf pans and let rise again, until dough crests the top of the pans. 10.
